At Fort Worth Snoring & Sleep Center, we are committed to helping the people of Fort Worth find solutions to their chronic TMJ pain and sleep problems using solutions custom-made for them. Your First Visit
Everything that happens during your first visit will be 100% tailored to why you came to see us. After being greeted by our front desk team, you’ll be escorted to a treatment room where you’ll meet Dr. Conditt. He’ll ask you what brought you to us, and after that, he’ll perform a comprehensive exam and may take some X-rays of your jaw, airway, and mouth. Once he’s finished, he’ll talk to you about what he sees and discuss all of your treatment options (not just the ones he offers). You’ll then have a chance to ask questions.
New Patient Forms
We know that our patients don’t have time to waste, so to help your appointment begin as quickly as possible, we suggest filling out our patient forms beforehand. New patient forms are provided to you directly by Dr. Conditt's office. If you have any issues accessing our forms, please call the office.
Payment Information
Our practice accepts cash, checks, and all major credit cards as forms of payment. If you would like to know whether or not you can use your insurance to offset the cost of your care, be sure to call ahead and talk to our team. We are contracted with most all medical insurances for treatment of obstructive sleep apnea. If you have questions about your particular insurance please feel free to contact our office. Depending on what you need, medical insurance can often provide coverage, but every plan is different.
